If you or a loved one is a patient in hospital and has received a discharge assessment or order, but have not been released, because the discharge is not organised, or a patient the needs more care after their interim care package ends.
The essential advocacy service package helps you ensure a timely discharge. By making sure you have a carer in place (if necessary), transport home from hospital arranged, a plan for preparing your home ready for your return to be picked up. make sure there is food and drink in the house and, if it is cold, put on the heating,. Also a check to make sure you have house keys with you, make sure you have suitable outdoor clothing to wear, and all your belongings and medication when you leave hospital.
