Creating Your Care Plan:
A Quick Guide to Self-Directed Assessments
Life changes quickly, and we have to change and adapt with the changes. As we move from stage to stage in our individual journeys, we need to learn how to roll with the punches and be prepared to live the best life possible,
Nowadays you don’t have to wait until a life altering illness changes how you operate and cope from day to day. Needing help is not unusual. Millions of people in the UK require care.
Approximately 500,000 adults live in care homes. Around 441,479 are elderly individuals, but approximately 20% of care home residents in England are under 65 years of age, with many residents receiving care due to disabilities or long-term health conditions.
There are approximately 4.7 million unpaid carers in England and approximately 310,000 unpaid carers in Wales;
Around 820,000 people receive domiciliary care in the UK. Domiciliary care providers include ‘hourly’ or ‘visiting’ home care, supported living, assisted living, or live-in care. These include patients who are ill, elderly, have long-term medical issues, or have been discharged from hospital care.
Around 100,000 people receive long-term support living, or assisted living care.
About 10,000 people need Live-in care.
In August 2024, around 400,000 people were awaiting assessment, review, or the start of a care service. Hospital discharges and assessments continue to be the biggest source of this care backlog and since 2019 the more people are leaving hospitals with more complex care needs.
If you live in the UK, you are hundreds of times more likely to need home care, then to have a car accident. Planning your hospital discharge strategy and planning for home care is just as important as any other insurance plan you can make for your life. If you already receive care, a review of you care plan could be beneficial, whether you, or the person you care for, need recovery, long-term or palliative care. Many people find domiciliary care to be a convenient option, many apply for a local authority care assessment because either they can’t afford the cost of living in a care home. Other reasons for considering home care include long waiting lists for community care and care homes, patients trying to avoid discharge delays and need to free up NHS hospital beds.
The care journey will most likely be challenging. It can also be very difficult to navigate the care system, as different local trusts have different care pathways and finding good options can be a lifeline.
The government in the UK spends inordinate amounts of money on Domiciliary care, and only about a quarter (23%) is self funded. A 2024 report found that adult social care (including home care) contributed over £68.1 billion to the economy in England – up by 13.2% since 2022/23. Domiciliary care provider services in England increased by 63%, from 8,414 in 2017 to 13,733 in 2024. By contrast, nursing home occupancy decreased by 5% in the same period (and by 11% for those without nursing). The adult social care domiciliary care sector is bigger than the home rental and food services industry, but demand is outstripping supply and people are still finding it difficult to access appropriate care services.
The Creating Your Care Plan PDF Guide, gives detailed information on different care options and re-ablement services, and explains how to self-direct your care planning and how to get the right support, to get the most from your care package. It is a useful resource for the carers and individuals planning for their care.
